Knee pain or kneecap pain?

Nick Smale asked The Guru for the following Physiotherapy Advice:

Hello,

I am 24 and have always been quite good at running. I am training for a marathon in October and have been doing more and longer runs than in the past (13-18miles every weekend). I am finding my knees are feeling increasingly painful/sensitive I’m guessing as a result of impact, they swell up a bit after runs and also click and grate abit when I flex them. I’m still ok to run, it’s just a bit uncomfortable and I’m worried I could be doing some serious damage. Any help would be much appreciated.

HI Nick

I think a good rule of thumb is that if it hurts, then it can’t be doing any good to you, long term.

This doesn’t mean your marathon days are over, but it does means that you have to find out why….

I’d have a look at your shoes and your running styled first up – go for the obvious! Then I’d look a the control you’ve got around your pelvis (on single knee squats) and see how much your big toe and ankle moves.

It sounds very kneecap orientated, possibly due to a tracking issue, but very, very treatable.

Perhaps time to see a good Physio?
